The Staff
The hospital staff are committed to assisting patients in rehabilitation. We appoint staff with expertise in rehabilitation or related fields to provide a program of care designed to meet the total needs of the individual.
Rehabilitation and Why You May Need It
Following surgery, an accident or a major illness, you may notice some reduction in your usual ability to cope with everyday activities. This can often be a very difficult experience for you and your family.
Rehabilitation provides the care, therapy and support needed to assist you towards attaining maximum independence.
At Calvary College Grove Rehabilitation Hospital we have teams of dedicated, specialist staff who provide a variety of rehabilitation programs including cardiac, orthopaedic, neurological (including stroke), multi-trauma, falls prevention, geriatric assessment, pulmonary and reconditioning. The programs are provided on an inpatient, day patient basis.
What does rehabilitation involve?
Assessment Prior to Admission
In most circumstance a Rehabilitation Assessment Nurse (RAN) will visit you prior to admission to undertake a Rehabilitation Assessment. This involves discussion and review of your current medical status. The information gained provides us with a baseline from which to plan your rehabilitation program.
On admission to Calvary College Grove Rehabilitation Hospital you will be assessed by a multidisciplinary team who will establish an individual plan of therapy in consultation with you. Together, realistic goals will be set.
A medical specialist in rehabilitation will oversee your therapy program and monitor your progress in conjunction with other team members. These team members are available to discuss your care throughout the program.
Depending on your needs, you may require assistance from any of the following team members:
- Medical Rehabilitation Specialist
- General Practitioner
- Nursing Staff
- Physiotherapist
- Occupational Therapist
- Speech Pathologist
- Dietitian
- Cardiac/Pulmonary Nurse
- Allied Health Assistants
Access to the following team members is also available if deemed necessary by your medical rehabilitation specialist – Clinical/Neuro Psychologist, Social worker, Podiatrist, Stomal Therapist, Orthotist
To achieve your rehabilitation goals you will need to be totally committed to the program and work with your team.
You and your family are an integral part of the rehabilitation program and you are encouraged to ask questions, raise issues and discuss your own personal needs with staff.
Who is responsible for my care?
If you are admitted for a rehabilitation program, a Medical Rehabilitation Specialist will co-ordinate your program and the team caring for you.
